A questionnaire used to assess a member's health and lifestyle habits. Once completed, an individual health report is sent to the member providing advice for health living and the future reduction of sickness or injury.

Medica Choice:
Medica Choice plans are favored by employers who want to significant health plan flexibility and broad access. Medica Choice products take advantage of Medica’s largest open access network. It includes nearly 27,000 providers of all types, at more than 4,000 offices, clinics and hospitals in Minnesota, Wisconsin, North Dakota and South Dakota.
Medica Elect Primary Care System:
Medica Elect is a focused network of leading care systems, providing quality health benefits at a more affordable price. Medica Elect members choose a care system and clinic to help coordinate care. Providers outside the care system are also available, at a lower benefit level. Urgent care/emergency care is open access, anywhere, anytime. Medica Elect can be offered alongside Medica Essential to provide employers with expanded care system options.
Medica Essential:
Medica Essential is a focused network of leading care systems, providing quality health benefits at a more affordable price. Medica Essential members choose a care system and clinic to help coordinate their care. Providers outside the care system are also available, at a lower benefit level. Urgent care/emergency care is open access, covered anywhere, anytime. Medica Essential can be offered alongside Medica Elect to provide employers with expanded care system options.
Medica Direct-Consumer-Directed Plans:
Medica Direct helps employers provide an affordable, quality benefit to employees. It includes essential coverage — for preventive care, catastrophic care, and prescriptions — and real choices for spending and saving. Medica Direct is available with HSA, HRA and FSA preferred financial accounts and in open access, care system and multi-state/national plan designs.
Passport from Medica—National/multistate 5-50 employees:
The Passport from Medica plan gives small employer groups (requires at least 5 employees) multistate coverage with impressive administrative efficiencies, managed care discounts and the potential for meaningful savings. It’s available in open access and consumer-directed plan designs.
